Proposal Request Forms
APS Pension and Financial Services, Inc. (APS) will prepare a pension plan proposal, profit sharing plan proposal, or a 401(k) plan proposal for a nominal fee (see below). If you establish the pension plan, APS will credit the nominal fee against the first year's administration fee. The proposal will take into account your benefit and funding objectives.
To submit your proposal request, either:
(1) Print out blank proposal request forms, complete them, and mail the completed forms along with your payment to APS,
OR
(2) (Coming Soon) Fill out the proposal request forms online, print them out, and mail the forms along with your payment to:
